Students who live in Burlingame, Hillsborough, or Millbrae are encouraged to complete a Disaster Service Worker (DSW) Registration Form for their city or town of residence. Please select and complete only the form for the community where you live.

Students who live outside the Central County Fire Department’s jurisdiction do not need to complete a DSW Registration Form.
